You will then receive an email from the team. It gives you a link to a DocuSign version of the Consent Form. Please take as much time as you need to read it.

You can choose to either complete the form electronically in advance, or sign a paper form on the day of the study. If there is anything that you are not clear about or find difficult to understand, please reach out to us via the addresses above.

You should only consent to participate in this research when you feel that you understand what is being asked of you, and you have had enough time to think about your decision.
